Overview
For decades, oral testosterone was considered off the table. Swallowed testosterone is broken down almost entirely by the liver before reaching circulation, and the modified versions developed to survive that — the 17-alpha-alkylated androgens — carried real liver toxicity.
Modern oral testosterone undecanoate products, including Kyzatrex, solved that differently. The testosterone is formulated so it is absorbed into the lymphatic system alongside dietary fat rather than passing straight through the liver. The liver toxicity of the old oral androgens does not apply.

Two things worth knowing before anything else. It carries a boxed warning for blood pressure increase — the FDA's most serious labeling category — and blood pressure must be controlled before starting and monitored throughout. And it must be taken with food, because absorption depends on dietary fat; taking it fasted substantially reduces how much reaches you.
Why oral testosterone was a problem for so long
Testosterone taken by mouth is absorbed from the gut into the portal vein and passes through the liver, which metabolizes almost all of it before it reaches the rest of the body. Very little arrives where it is needed.
The workaround developed decades ago was to chemically modify testosterone at the 17-alpha position so it resisted hepatic breakdown. It worked — and those compounds, methyltestosterone among them, carried genuine liver toxicity including cholestasis and, with prolonged use, rare but serious liver tumors. They fell out of favor for good reason, and the reputation attached itself to the whole category.
Testosterone undecanoate is a different approach. Attaching a long fatty acid chain makes the molecule lipophilic enough to be absorbed into intestinal lymphatic vessels along with dietary fat. The lymphatic system drains into the general circulation without first passing through the liver. First-pass metabolism is bypassed by route rather than by chemical modification, so no 17-alpha alkylation is needed.
This is the same principle behind why a transdermal estrogen behaves differently from an oral one — bypassing the liver changes the profile.
The blood pressure warning
This is the most important thing on the page and it should not be softened.
Oral testosterone undecanoate products carry a boxed warning — the FDA's strongest — for increases in blood pressure, which raise the risk of major cardiovascular events including heart attack and stroke.
What follows from it in practice:
Blood pressure should be controlled before starting. Uncontrolled hypertension is a reason to address that first rather than to proceed and monitor.
It is measured on treatment, not assumed. This is the specific monitoring requirement that distinguishes oral testosterone from other routes.

The food requirement, which is not a formality
Absorption depends on being taken with food containing fat. That is not a tolerability suggestion — it is the delivery mechanism.
Taken on an empty stomach, absorption drops substantially and levels can fall well below what the regimen intends. Men who take it fasted and then report that it is not working are frequently describing an absorption problem rather than a treatment failure.
It is taken with meals, on whatever schedule you are prescribed. Consistency in how it is taken matters as much as consistency in taking it, which is a difference from most medications and is worth understanding at the start.

Testosterone products, including this one, are indicated for men with low testosterone due to identified medical conditions — problems with the testicles, pituitary or hypothalamus. They are not indicated for low testosterone due to aging alone, and the labeling says so.
