Hematocrit: Why It Gets Watched on Hormone Therapy

Overview

Hematocrit is the proportion of your blood made up of red cells. It is a routine number on a blood count and, for anyone on testosterone therapy, it is the single most important safety measurement being monitored.

Testosterone stimulates red cell production. That is a known, expected, dose-related effect — not a side effect in the unusual sense — and it is the reason a blood count is checked before starting and repeatedly afterward.

The reason it is watched: thicker blood flows less easily, and marked erythrocytosis is associated with increased clotting risk. The absolute risk at modestly raised levels is debated, and the monitoring is not — checking a blood count is cheap, and the alternative is not knowing.


Why testosterone raises it

Testosterone increases red cell production through more than one route: it stimulates erythropoietin, the kidney hormone that drives red cell formation, and it appears to alter the set point at which erythropoietin production is regulated. It also affects iron availability by suppressing hepcidin, freeing up more iron for red cell production.

Two practical consequences follow.

Route matters. Formulations producing higher peak levels tend to produce more erythrocytosis than those delivering steadier levels. Injections given less frequently, at larger amounts, produce the highest peaks. How oral and injectable differ.

Dose matters, which is one reason a regimen is reviewed rather than simply continued when hematocrit climbs.

Onset is typically within the first several months, which is why monitoring is front-loaded rather than annual.

What is being watched for

The concern is blood viscosity. As the proportion of red cells rises, blood becomes thicker and flows less easily, and above a certain point that is associated with increased risk of clotting events.

Two honest points about the evidence. The threshold commonly used in practice — around 54% — is derived partly from other conditions and from expert consensus rather than from randomized trials of testosterone therapy specifically. And the absolute risk at levels modestly above the reference range is genuinely debated.

What is not debated is the monitoring. The number is cheap to measure, the trend is informative, and finding it early gives options that finding it late does not.

Symptoms of significant erythrocytosis — headaches, flushing, dizziness, visual disturbance, itching after a hot shower — are worth knowing, though many men have none, which is precisely why it is measured rather than waited for.

What else raises it

Important, because attributing a high hematocrit to testosterone when something else is driving it means missing the something else.

Obstructive sleep apnea is the big one. Intermittent overnight low oxygen drives red cell production, and it is common, under-diagnosed, and disproportionately present in exactly this population. A man on testosterone with a rising hematocrit should be screened for it — and testosterone can worsen apnea, so the two interact. Why apnea gets missed.

Smoking, through carbon monoxide exposure. One of the most modifiable contributors.

Dehydration, which raises the measured proportion without any change in red cell mass. A common reason for a spuriously high single reading, and a reason to repeat before acting.

Chronic lung disease and living at altitude, both through the same oxygen mechanism.

Diuretics, through fluid loss.

Polycythemia vera, a bone marrow disorder — uncommon, and the reason an unexplained persistently high hematocrit in someone not on testosterone is investigated rather than assumed.

What happens if it rises

This is a clinical decision and the specifics depend on the level, the trend, the symptoms and the rest of the picture. What is generally involved:

Confirming it — repeating the test, properly hydrated, since a single reading can mislead.

Looking for the other causes above, particularly sleep apnea and smoking, because addressing those sometimes resolves it without changing the regimen.

Reviewing the regimen — the amount, the frequency and the route, since steadier delivery tends to produce less erythrocytosis than large infrequent peaks.

Therapeutic phlebotomy in some cases, which removes blood to lower the proportion directly. It is used, and it is not a first-line answer to a problem that has a modifiable cause — repeated phlebotomy also depletes iron over time.

Pausing or stopping treatment where the level is high enough or does not respond.

What we would not do is continue unchanged and keep measuring. Monitoring exists to inform a decision, not to substitute for one.
