Overview
The active hormone is the same. What differs is the path it takes to your bloodstream, and that turns out to matter more than most people expect.
A pill is absorbed through the gut and passes through the liver before reaching general circulation. That first pass through the liver changes how the liver behaves — it alters production of clotting factors, binding proteins and triglycerides. A patch or gel is absorbed through the skin and enters circulation directly, so the liver sees ordinary circulating levels rather than a concentrated first pass.

Where guidance has landed: for women with any elevated clot risk — higher body weight, previous clot, migraine with aura, some clotting conditions — transdermal is generally preferred. For women without those factors, both are reasonable, and the choice comes down to what fits your life and what you tolerate.
What the liver has to do with it
This is the mechanism underneath every difference in the table, so it is worth understanding once.
When a hormone is swallowed, it is absorbed from the gut into the portal vein, which runs directly to the liver. The liver metabolizes a substantial fraction before anything reaches the rest of the body — and in doing so it is exposed to a much higher concentration than any other tissue.
The liver responds by changing its output. Production of clotting factors increases. SHBG production increases substantially. Triglyceride production increases. C-reactive protein rises. None of these is a side effect in the usual sense; they are the liver responding normally to a signal it would not otherwise receive at that concentration.
Absorbed through skin, estradiol enters the general circulation and reaches the liver at ordinary concentrations, alongside everything else. The hepatic effects are largely absent.
What that means for clot risk
This is the clinically significant difference and the reason the route question matters at all.
Observational studies consistently find an increased risk of venous thromboembolism with oral estrogen and no detectable increase with transdermal estradiol at standard doses. The mechanism — hepatic clotting factor production — explains why, which strengthens the case considerably beyond association alone.
The practical consequence: for women with anything that already raises clot risk, transdermal is the route generally recommended. That includes higher body weight, a previous venous clot, a known thrombophilia, migraine with aura, and periods of immobility such as major surgery.
Two caveats worth keeping in proportion. Baseline venous clot risk in healthy women in this age range is low, so a relative increase on oral treatment is still a small absolute number for most people. And the studies are observational rather than randomized head-to-head, because the trial that would settle it definitively has not been done and probably will not be.
The SHBG effect, which surprises people
Oral estrogen raises SHBG substantially — a genuinely large effect.
Since SHBG binds testosterone and only the unbound fraction is active, more SHBG means less free testosterone. For a woman whose libido or energy is already a concern, that is a real and under-discussed consideration. It also means that a testosterone result taken while on oral estrogen is interpreted differently from one taken on a patch. What SHBG does and what moves it.
This is a consideration rather than a rule against oral treatment. It is worth knowing about before it becomes a puzzle later.
Where each route is genuinely the better fit
Oral is simple, familiar, and nothing is visible or falls off. Some women simply prefer a tablet, and adherence is what makes any treatment work. It has a modestly favorable effect on HDL cholesterol — though the significance of that is less clear than it once seemed. It is also generally cheaper.
Transdermal avoids the hepatic effects, delivers steadier levels without daily peaks and troughs, and does not require gut absorption — which matters after bariatric surgery or with malabsorption. Patches are changed once or twice weekly; gel is daily and needs to dry before contact with others.
The drawbacks are practical rather than clinical: patches can irritate the skin, and adhesion varies with heat, humidity, swimming and exercise. Gel avoids the adhesive problem and introduces a transfer one.

Frequently asked questions
Is the patch safer than the pill? For venous clot risk specifically, the evidence favors transdermal, and it is generally preferred for women with any elevated clot risk. For a woman without those risk factors, both are reasonable options.
Why does the route matter if the hormone is the same? Because a swallowed hormone passes through the liver first, at high concentration, and changes what the liver produces — clotting factors, SHBG and triglycerides among them. Absorbed through skin, it does not.
Can I switch between them? It is a common change and it is a clinical decision, since the equivalence between routes is not one-to-one. It is worth raising specifically rather than adjusting independently.
Do patches always stick? Adhesion varies with heat, humidity, swimming and exercise. Gel is an alternative for anyone who finds patches unreliable.
Which is better for my libido? Oral estrogen raises SHBG and lowers free testosterone, which is worth factoring in if libido is a concern. It is one consideration among several rather than the deciding one.
