Overview
Sex hormone-binding globulin is a transport protein made in the liver. It binds testosterone and estradiol and carries them through the blood — and while a hormone is bound to it, it is not available to your tissues. So SHBG effectively decides how much of your total testosterone is actually doing anything.
That is why it belongs on the panel. Without it, a total testosterone result can be genuinely misleading in both directions.

The one-line version: total testosterone tells you how much there is. SHBG tells you how much of it counts.
What SHBG does
Think of it as a courier. Testosterone and estradiol circulate mostly bound — to SHBG tightly, and to albumin loosely. Only a small fraction circulates completely unbound, and it is that free fraction, plus the loosely bound portion, that can actually enter tissue and act.
Because SHBG binds tightly, more SHBG means less available hormone from the same total. Less SHBG means more available hormone from the same total. Nothing about your production has changed; the availability has.
This is why measuring total testosterone alone can produce a result that does not match how someone feels. It is one of the more common reasons a man is told his testosterone is normal and leaves the appointment no better informed than when he arrived.
What the ranges look like
Most laboratories report reference ranges in the region of 10–57 nmol/L for men and 18–144 nmol/L for women, with substantial variation between laboratories and assays and a wide spread by age. Women run higher than men, and SHBG tends to rise gradually with age in both.
As with every marker on a panel, these are reference values describing a population — not personal targets. A result near the top of range means something different in a lean man on no medication than it does in someone with a thyroid abnormality or on oral estrogen.
Why it goes up
Age. SHBG drifts upward through midlife in both sexes, which quietly reduces available testosterone even when total production is unchanged. This is one of the reasons symptoms and totals diverge with age.
Thyroid overactivity. Hyperthyroidism raises SHBG, sometimes markedly.
Oral estrogen. Anything estrogenic taken by mouth passes through the liver first and increases SHBG production. This is the single most common medication-related cause and it is highly relevant in midlife women — the route matters, which is part of why we discuss patch versus pill as a real clinical choice rather than a preference.
Liver disease. The protein is made in the liver, so liver function affects output.
Being lean, and training hard. Low body fat and high training volume both tend to raise SHBG. This is worth knowing if you have an endurance background, because it is one of several reasons a lean former athlete can show a picture that looks like low testosterone.
Some anticonvulsants and other medications.
Why it goes down
Insulin resistance and higher body weight. This is the most common cause of a low SHBG in the people we see, and it makes SHBG useful as an indirect signal of metabolic health rather than only a hormone transporter. A low SHBG on a panel is often the first hint of insulin resistance that has not yet shown up on glucose.
Hypothyroidism, the mirror of the finding above.
Corticosteroids and some other medications.
Fatty liver.
Which means SHBG is one of those results that tells you about more than the thing it was ordered for. A low value in someone with a rising waist measurement and a creeping fasting glucose is a coherent picture, and it is a metabolic conversation rather than a hormonal one.
How it changes what gets measured
If SHBG is at either extreme, total testosterone stops being the right number to reason from. What matters then is free testosterone, or a calculated free testosterone derived from total testosterone, SHBG and albumin.
Calculated free testosterone is the practical option in most settings, and it is reasonably reliable when the inputs are good. Directly measured free testosterone by equilibrium dialysis is the reference method, less widely available, and generally reserved for cases where the calculation and the clinical picture disagree.
The point for anyone reading their own results: if your total testosterone is normal but you have symptoms, ask what your SHBG is. It is one of the more common explanations for that mismatch, and it is a straightforward thing to check.
Frequently asked questions
Can I lower my SHBG? Where the cause is modifiable — insulin resistance, thyroid dysfunction, oral estrogen, a medication — addressing the cause tends to move it. There is no useful way, or reason, to target the number itself in isolation.
Is high SHBG bad? Not inherently. It becomes clinically relevant when it is high enough that available testosterone falls despite a normal total, and the person has symptoms. On its own it is information, not a problem.
Why did my testosterone come back normal when I feel awful? SHBG is one of the more common explanations, though not the only one. Thyroid function, iron studies, sleep quality and mood all produce an overlapping symptom picture, which is why a full panel is more useful than a single hormone.
Does SHBG matter for women? Yes, and it is less often discussed. It affects available testosterone and estradiol in women too, and oral estrogen raises it substantially — which can be relevant when someone on hormone therapy is not getting the response they expected.
