What it is and how it works
Most treatment for erectile difficulty is organized around an event. You take something, you wait, and the clock is running.

It relaxes the smooth muscle in blood vessel walls, which increases blood flow into the penis in response to arousal, and it does the same thing in the prostate and bladder neck. It has been FDA-approved for erectile dysfunction since 2003 and separately for the urinary symptoms of benign prostatic hyperplasia since 2011, and it is available as an inexpensive generic. The distinguishing feature is how long it lasts — the effect persists well beyond a day, which is what makes a daily tablet practical where shorter-acting alternatives are taken in anticipation of sex. Tadalafil is a PDE5 inhibitor, and PDE5 inhibitors interact dangerously with nitrates.
Do not take this if you use any nitrate medication. That includes nitroglycerin in every form — tablets, spray, patches — isosorbide mononitrate or dinitrate, and recreational amyl nitrite, commonly sold as poppers.
The combination can cause a sudden, severe drop in blood pressure that can be life-threatening. This is not a risk to be balanced against a benefit; it is an absolute contraindication. If you carry nitroglycerin for angina, say so at assessment — and tell any emergency clinician that you have taken a PDE5 inhibitor, because it changes what they can safely give you.
Alpha-blockers deserve separate mention in this age group. They are commonly prescribed for prostate symptoms, they lower blood pressure by a different route, and the combined effect can cause dizziness or fainting. It is usually manageable with care over timing and sequence, but it is a conversation to have before the first tablet rather than after.
Why daily, rather than when you need it
The same medication can be taken either way, and the choice is practical rather than clinical in most cases.
As-needed dosing is taken in anticipation. It works, it is well established, and where sex is occasional it is the cheaper option by a wide margin. What it asks is that you anticipate, which some couples find unobjectionable and others find corrosive.
Daily dosing removes the anticipation. There is no window to be inside and nothing to remember at the wrong moment, and for men having sex regularly the arithmetic often favors it anyway. It is also the form used for prostate symptoms, so a man with both problems gets both addressed by one tablet.
Neither is a better drug. Daily versus as-needed works through the comparison honestly, including the case for not choosing daily.
The symptom that is worth investigating
Erectile difficulty is a vascular event as often as it is anything else, and the arteries involved are narrower than the coronary arteries. When atherosclerosis is developing, they tend to show it first.
That makes new erectile difficulty in midlife a useful early signal, and it is the reason a prescription on its own is an incomplete answer. Diabetes, high blood pressure, lipid problems, sleep apnea and low testosterone all present this way, and all of them matter more than the symptom that brought them to attention.
A tablet that restores function also removes the prompt that would have led to any of those being found. What it will not do
It will not create desire. PDE5 inhibitors act on blood flow in response to arousal, and where arousal itself has faded there is nothing for them to amplify. Men who expect otherwise conclude the drug failed when it was never the right target.
Where desire is the complaint, the list to work through is testosterone, sleep, mood, alcohol, medication side effects — antidepressants and some blood pressure drugs in particular — and what is happening in the relationship. Testosterone therapy addresses a genuine cause when it is the cause, and the Triple Action Blend exists because desire and blood flow are different pathways that sometimes both need attention.
It will also not work for everyone. Non-response is a finding worth following rather than a failure of the prescription, and it usually points somewhere specific.

It suits men less well if sex is infrequent, where an as-needed tablet is cheaper and does the same job. It is not appropriate at all for anyone taking nitrates. And it treats erections, not desire: if what has gone is interest rather than function, the question is a different one, and testosterone or the desire pathway addressed by the Triple Action Blend is the more relevant conversation.
