Compounded ED Troches vs Pills: How the Form Differs

Onset, somewhat. Absorption through the mouth begins within minutes, so the earliest part of the drug's effect can arrive sooner than with a swallowed tablet that must empty from the stomach first. For tadalafil, whose swallowed form already reaches useful levels well within an hour and lasts more than a day, the difference is modest. For the peptide components it is the point of the form, covered below.

Food, somewhat. A swallowed tablet taken after a large or fatty meal is absorbed more slowly; the mouth-absorbed fraction of a troche is not subject to that. Again, tadalafil is the drug in the class least affected by food in the first place, so the advantage is smaller than the marketing implies.

The liver, partly. Drug absorbed through the mouth lining skips the first pass through the liver. For tadalafil that matters little — the swallowed drug is absorbed well — but it is the reason troches are used for molecules the gut would otherwise destroy.

Taste and convenience. A troche can be held discreetly and does not need water. It also tastes of whatever the base is, and some men find the slow dissolve unpleasant. It is a preference, and preferences are allowed.

The drug. Tadalafil in a troche is the same tadalafil as in a tablet, with the same duration, the same cautions, the same nitrate rule and the same blood-pressure interactions. A troche does not make it safer, does not remove the interactions, and does not make a man who should not take tadalafil into one who can.

The effect. A PDE5 inhibitor works on blood flow in response to arousal, in whatever form it arrives. The troche does not produce an erection without stimulation any more than the tablet does, and claims that a sublingual form is "stronger" describe absorption speed, not effect.

The evidence. The trials that established tadalafil are trials of the tablet. The troche form has no published trials of its own; its use rests on the pharmacology of the tablet plus the general behavior of buccal absorption.

Because two of the three drugs cannot be swallowed. PT-141 and oxytocin are peptides — chains of amino acids — and the stomach digests peptides as it digests any protein. Given as a tablet, they would be broken down before reaching the bloodstream. PT-141 is approved as an injection for exactly this reason; oxytocin is given by injection or nasal spray. The troche is a way of delivering some of each through the mouth lining, where digestive enzymes are absent, without a needle.

That is the genuine rationale for the form, and it comes with the honest caveat: how much PT-141 and oxytocin actually cross the mouth lining from a compounded troche has not been measured. The main page is candid that the combination is not first-line and that its evidence is thinner than tadalafil's alone. The troche is the practical answer to "how do you give three drugs, two of them peptides, in one product without an injection"; it is not evidence that the two peptides arrive in useful amounts.
