When in Your Cycle to Test Hormones — and Why It Matters

Overview

Timing matters enormously for some hormones and barely at all for others, and the confusion between the two costs people money and produces results that cannot be interpreted.

There is also a harder truth underneath the timing question: in perimenopause, hormone levels swing so widely from week to week that a single draw often cannot tell you much at all. That is not a failure of the test. It is the actual physiology of the transition, and it is why perimenopause is diagnosed clinically — from your cycle pattern and symptoms — rather than from a blood result.

The short version: if you are still having periods, the cycle-sensitive hormones should be drawn on a stated cycle day and the day should appear on the report. If your cycles have become unpredictable, timing becomes an approximation, and the panel becomes supporting evidence rather than an answer.


Why timing matters so much for three of them

Estradiol, progesterone, FSH and LH are not stable values you happen to sample. They are a sequence, and each one means something different depending on where in that sequence it was drawn.

Estradiol is low in the early follicular phase, climbs steeply toward ovulation, drops, then rises again in the luteal phase. The same woman can produce a very low reading and a very high one within a fortnight, both entirely normal. Drawing it on a stated early-follicular day gives a comparable baseline; drawing it at random gives a number with no denominator.

Progesterone is the starkest case. It is essentially at floor level for the whole first half of the cycle and rises only after ovulation. A progesterone drawn in the follicular phase is not "low" — it is simply the wrong day. This is the most common avoidable error we see on results people bring us.

The old shorthand for progesterone was "day 21," which works only if you ovulate on day 14 of a 28-day cycle. The convention that actually holds is roughly seven days after ovulation, which in a 32-day cycle is closer to day 25.

FSH rises as ovarian reserve declines, which is why it appears in menopause discussions. It is also the hormone most misused, for reasons below.

Why FSH does not diagnose perimenopause

This is the single most useful thing on this page.

FSH fluctuates dramatically during the transition — high one month, ordinary the next, sometimes within the same cycle. Someone squarely in perimenopause can produce a completely unremarkable FSH, and someone with a high FSH can go on to have several more ovulatory cycles.

Major menopause guidance is consistent on the point: for women over roughly 45 with typical symptoms and changing cycles, the diagnosis is clinical and hormone testing is not required. A single FSH cannot confirm perimenopause and cannot rule it out.

Two situations where it genuinely earns its place: suspected early menopause under 40 or 45, where confirming the picture changes management substantially, and cases where something else — thyroid disease, high prolactin, pregnancy — needs to be excluded.

Hormonal contraception suppresses the whole axis, which makes FSH uninterpretable while it is in use.

What is worth testing, even when the sex hormones are not

The most productive panel in perimenopause is frequently the one that does not measure sex hormones at all — because the symptoms overlap almost completely with several conditions that a blood test can settle.

Thyroid function, because hypothyroidism peaks in this exact age group and produces fatigue, weight change, low mood and cold intolerance. The three thyroid tests and what each does.

Ferritin, because heavier and more frequent bleeding is characteristic of the transition, and iron depletion is the most commonly missed cause of perimenopausal fatigue.

B12 and vitamin D, both common and both easy to correct.

Glucose, HbA1c and a lipid panel, because cardiovascular risk changes measurably through this window and it is the moment when a baseline is most useful.

Those results are stable and interpretable on any day of any cycle. The sex hormones are the ones that need care.

Practical points that change a result

Biotin — common in hair and nail supplements — interferes with many immunoassays and can distort thyroid and some hormone results in either direction. Pause it before testing and tell the laboratory you take it.

Time of day. Testosterone and cortisol are highest in the morning. A late-afternoon testosterone in a man will read meaningfully lower than the same man's 8am draw, which is why morning collection is standard.

Same laboratory, same assay. Hormone assays vary more between laboratories than most people assume. If you are tracking a value over time, tracking it at one lab is worth more than a technically better result somewhere else.

Record the cycle day. A result without a cycle day attached cannot be fully interpreted later, even by the person who ordered it.

Frequently asked questions

What day should I test estrogen and progesterone? Estradiol on roughly days 2 to 5 if you are still cycling; progesterone about seven days after ovulation. If your cycles have become irregular, exact timing is no longer possible and the results should be read as an approximation.

Can a blood test tell me if I am in perimenopause? Not reliably. Over 45, with typical symptoms and changing cycles, the diagnosis is made clinically. Testing is more useful for excluding thyroid disease, anemia and other explanations than for confirming the transition.

My FSH was normal. Does that rule out perimenopause? No. FSH fluctuates enough during the transition that a normal value is common and does not exclude it.

Does hormonal contraception affect the results? Yes, substantially. It suppresses FSH, LH and your own estradiol production, which makes those values uninterpretable while you are taking it.

Should I test at all if the diagnosis is clinical? Often yes, but for a different reason — to check the things that mimic perimenopause and to establish a metabolic and bone-health baseline at the moment it becomes most relevant.

If your cycles are changing and you feel different, the useful sequence is: describe the pattern, rule out the mimics with a panel that does not depend on cycle timing, and treat sex-hormone levels as supporting information rather than the verdict.
