Overview
Ferritin is the protein that stores iron, so a high result is often read as "too much iron." Sometimes that is right. More often it is not, because ferritin also rises with inflammation, regular alcohol, liver strain, obesity and recent illness — entirely independently of how much iron you are carrying. Which is why a raised ferritin is a question rather than an answer.

The single most useful thing to know: ferritin alone cannot distinguish these. Transferrin saturation is the test that separates real iron overload from everything else, and it is the number to look for alongside it.
What ferritin actually is
Ferritin is a storage protein. Its job is to hold iron in a safe, bound form until the body needs it, and the amount circulating in your blood broadly reflects how much iron is in store. That is why it is the standard first-line test for iron status — it usually falls before you become anemic, which makes it an early warning for depletion. We cover that direction in low ferritin.
The complication is that ferritin has a second job. It is an acute-phase reactant: a protein the body produces more of during inflammation, infection and tissue injury. That is a useful biological response and an inconvenient one for interpretation, because it means ferritin can be high for reasons that have nothing to do with iron.
Where the ranges sit, and why they are only a starting point
Most laboratories report reference ranges of roughly 24–336 ng/mL for men and 11–307 ng/mL for women, though these vary meaningfully between laboratories and assays. Post-menopausal women often sit higher than pre-menopausal women simply because monthly iron loss has stopped.
Those are reference values, not targets. A result at the top of range in someone with normal transferrin saturation and no symptoms means something quite different from the same number in someone with joint pain, a family history of hemochromatosis and raised liver enzymes.
The five reasons it goes up
Iron overload. The one everyone worries about, and the least common. Hereditary hemochromatosis is the main inherited cause, and it matters because it is treatable and because untreated iron accumulation damages the liver, heart, pancreas and joints over decades. It is more prevalent in people of Northern European ancestry. The tell is that transferrin saturation is high too — typically above about 45%.
Inflammation. Any inflammatory process will lift ferritin. Autoimmune conditions, chronic infection, recent surgery, even a heavy cold the week before your draw. Here transferrin saturation is normal or low, which is the giveaway.
Alcohol. Regular drinking raises ferritin, and this is one of the more common explanations in midlife. It is also one of the more under-reported, because social and business drinking rarely gets counted. It often improves substantially within weeks of reducing intake.
Fatty liver and metabolic strain. Raised ferritin travels with insulin resistance, higher body weight and non-alcoholic fatty liver disease. In this context it is better understood as a marker of metabolic stress than of iron.
Recent illness or injury. Transient, and the reason a repeat test a few weeks later is often the most informative next step rather than any intervention.
What we do with a high result
The sequence is straightforward and it starts with not panicking.
First, look at transferrin saturation, and at total iron and TIBC if they are available. If ferritin is high and saturation is normal, iron overload is unlikely and the question becomes what else is driving it.
Second, look at the inflammatory and liver markers on the same panel — hs-CRP, ALT, AST, GGT. A pattern of raised ferritin with raised liver enzymes in someone carrying extra weight points somewhere quite specific.
Third, take an honest alcohol history. Not a moral one, a factual one. This is the single most common modifiable contributor in the age group we work with.
Fourth, repeat it. Ferritin moves. A result taken three weeks after a chest infection is not your baseline.
Where the picture genuinely suggests iron overload — high ferritin, high transferrin saturation, relevant family history — that is a referral for genetic testing and specialist assessment, not something to manage through a wellness panel.
What high ferritin does not mean
It does not mean you should stop eating iron-containing foods. It does not mean you need a supplement of any kind, and it certainly does not mean you should take iron. And it is not, on its own, a reason to donate blood as a treatment — therapeutic phlebotomy is a genuine treatment for genuine iron overload, prescribed and monitored, not a self-directed intervention.
Anything that involves removing iron from your body is a clinical decision made on a full picture, not on a single number.
Frequently asked questions
Is high ferritin dangerous? It depends entirely on the cause. Sustained genuine iron overload damages organs over years and is worth identifying. Ferritin raised by inflammation or alcohol is a signal about something else, and the something else is what gets treated.
Should I give blood to bring it down? Not as a self-treatment. Therapeutic phlebotomy is a real treatment for confirmed iron overload, done on a schedule and monitored. Donating blood to move a number you have not had properly interpreted is not the same thing.
Can I lower ferritin through diet? If the driver is alcohol or metabolic, then yes — reducing alcohol and improving insulin sensitivity both help, sometimes substantially. If the driver is inherited iron overload, diet will not fix it.
My ferritin is high but I feel fine. Does it matter? Possibly. Iron overload is often silent for years before it causes damage, which is exactly why it is worth ruling in or out properly rather than ignoring. Equally, a mildly raised ferritin from inflammation in someone who feels well may need nothing more than a repeat test.

Ferritin is one of the results most often misread in both directions — treated as alarming when it is inflammatory, and dismissed when it is not. The useful move is to read it alongside transferrin saturation, the liver and inflammatory markers, and an honest account of your drinking.
