High Homocysteine: What It Signals and What It Does Not

Overview

Homocysteine is an amino acid your body produces and then clears, using B12, folate and B6 to do it. When one of those is short — or when kidney function is reduced — homocysteine accumulates. So a raised result is usually a signal about your B vitamin status or your kidneys, and its main value is as a pointer rather than as a problem in its own right.

That distinction matters, because the evidence for treating the number itself is much weaker than the marketing around it suggests.

The key point: homocysteine is worth measuring because of what it reveals. Lowering the number is not itself a demonstrated benefit.


What it is and where it comes from

Homocysteine sits in the middle of a metabolic pathway. Your body makes it from methionine, an amino acid from dietary protein, and then converts it onward — either recycling it back to methionine, which needs B12 and folate, or converting it down a different route, which needs B6.

If any of those cofactors is in short supply, the traffic backs up and homocysteine rises in the blood. That is the whole mechanism, and it is why the marker functions as an indirect readout of B vitamin sufficiency.

Kidneys clear homocysteine, so reduced kidney function raises it independently of any vitamin issue. This is a common and often overlooked explanation, particularly in older adults.

Where the ranges sit

Most laboratories report reference ranges of roughly 5–15 µmol/L, varying by laboratory and assay, with levels drifting upward with age and running slightly higher in men.

The usual caveat applies with particular force here. There is genuine debate about where a "desirable" level sits versus a statistically normal one, and various tighter targets circulate in the functional medicine space. We would rather not print one as though it were settled, because the underlying question — whether pushing homocysteine lower improves outcomes — is exactly the thing the evidence has struggled to demonstrate.

What the evidence actually shows

This is the part usually left out, and it is the most useful thing on this page.

Homocysteine is consistently associated with cardiovascular disease, stroke and cognitive decline. That association is real and it is reproducible.

But large randomized trials that lowered homocysteine using B vitamins largely failed to reduce cardiovascular events. The number came down; the outcomes did not follow. That result has been replicated across several major trials, and it is the reason mainstream guidelines do not recommend routine homocysteine screening or B vitamin supplementation for cardiovascular prevention.

The most likely explanation is that raised homocysteine is a marker of an underlying process rather than a cause of it — a signal rather than a lever. There remains some interest in specific contexts, including cognitive outcomes in people with genuine B12 deficiency, but the general case for treating the number has not held up.

So the honest framing is: worth measuring for what it tells you about B12, folate and kidney function. Not worth chasing as a target.

The MTHFR question

This deserves its own paragraph because it is among the most oversold tests in consumer health.

MTHFR variants affect an enzyme in folate metabolism, and common variants are genuinely common — a substantial share of the population carries at least one copy. Having one can modestly raise homocysteine.

What it does not do is explain fatigue, anxiety, brain fog, miscarriage or the long list of things it gets blamed for on the internet. The clinical significance of the common variants in someone with adequate folate intake is limited, and the finding rarely changes management.

What we do with a raised result

Look at the causes, not the number.

Check B12 and folate. If either is low, that is the finding — and B12 deficiency in particular is worth identifying because it has neurological consequences if it persists. Repletion is a clinical decision, not something to self-direct from an article.

Check kidney function, which is already on a standard panel.

Check thyroid function, since hypothyroidism raises homocysteine.

Review medications. Long-term metformin reduces B12 absorption, which is a common and quiet cause in exactly the population taking it for metabolic reasons.

Take an honest alcohol history, which affects both B vitamin status and the result directly.

And then treat what you found, rather than the homocysteine.

Frequently asked questions

Should I take B vitamins to lower my homocysteine? Only if you are deficient in one, in which case you are treating the deficiency and the homocysteine follows. Supplementing to lower the number in someone with adequate B vitamin status has not been shown to improve outcomes.

Does high homocysteine mean I will have a heart attack? No. It is associated with higher risk at a population level, and lowering it has not reduced events in trials — which suggests it is reporting on something rather than causing it.

I have an MTHFR variant. Does that explain my symptoms? Almost certainly not on its own. The common variants are widespread and their clinical impact in someone with adequate folate is limited.

Is this test worth having? It is a reasonable inclusion on a broader panel, read as a pointer toward B12, folate and kidney function. As a standalone test to monitor and lower, considerably less so.

Where this fits in your plan

Homocysteine is a useful signal and a poor target. If it comes back raised, the productive move is to look at what could be driving it — B12, folate, kidneys, thyroid, medication, alcohol — and address whichever of those turns out to be real.

That is the kind of reading a full panel makes possible, and the reason we run these together rather than one at a time.
