Client Dinners, Conferences, and the Drinking Nobody Counts

Why this pattern hides

Almost every framework for thinking about alcohol is built around excess: binge drinking, dependence, the obvious problem. The executive pattern is none of those. It is structural — dinners, receptions, board weekends, a glass while the inbox clears — and because no single occasion looks like much, the weekly total is rarely added up.

The second reason it hides is that the consequences are non-specific. Poorer sleep, flatter mornings, a bit more weight around the middle, liver enzymes drifting toward the top of normal. Every one of those has an alternative explanation available, and age is usually the one that gets used.

What it moves

Sleep, first and most. Alcohol shortens the time it takes to fall asleep and degrades the second half of the night, fragmenting sleep and reducing its restorative quality. This is why the drinking pattern and the exhaustion complaint so often belong to the same person, and why it goes unconnected: the sleep felt fine going in.

It also worsens sleep-disordered breathing, which matters given how commonly that goes undiagnosed in this group. If you have both, they compound.

Liver enzymes. GGT and the transaminases respond to regular intake at volumes well below what anyone would describe as heavy — and the effect is more pronounced at higher body mass index. This is one of the more common findings on a first panel in this population, and one of the most reversible.

Hormones. Alcohol affects testosterone synthesis in men, and the relationship is dose-dependent. In women, it is a well-recognized trigger for vasomotor symptoms during the menopausal transition — hot flashes and night sweats — which means it stacks with the sleep effect at exactly the age it lands hardest.

Weight and glucose. Drinks carry calories that displace nothing and tend to be paired with food chosen late in the evening. The effect on morning glucose is often visible.

Blood pressure. Regular intake raises it, and reduction is one of the standard non-drug recommendations for lowering it.

The reversible part

The clinically useful feature of all of this is that it is not a slow accrual of permanent damage at these volumes. It is a set of ongoing effects that track current intake. Liver enzymes move within weeks of a change. Sleep quality — the actual restorative part, not the time asleep — is often the fastest thing to improve and the most noticeable. Vasomotor symptoms in perimenopausal women frequently improve. Blood pressure responds.

Which is why the productive version of this conversation is not a lecture. It is a measurement, a change for eight to twelve weeks, and a second measurement, so you are looking at your own data rather than at general advice.

What we do, and what we are not

We ask about alcohol directly on intake, we measure the markers it affects, and we tell you what we see without moralizing about it. Where the picture suggests intake is driving a finding, we say so, and we would rather test that hypothesis with a temporary change and a repeat panel than treat the downstream number.

We are not an alcohol treatment program. If the picture is one of dependence — drinking to prevent withdrawal, unsuccessful attempts to cut down, drinking that continues despite clear harm — that needs proper care, and we will say so and refer rather than manage it around the edges. Withdrawal from heavy regular drinking can be medically dangerous and is not something to attempt on your own.

Add up an actual week — not a typical week, last week — and be straight about it on intake. Then get a baseline panel that includes liver function, and use it as a starting point rather than a verdict.

  • U.S. Dietary Guidelines describe moderate drinking as up to one drink a day for women and up to two for men, and are explicit that less is better and that this is not a recommendation to start. The pattern matters too — the same weekly total taken late at night, close to sleep, behaves differently.

  • They can be, particularly at higher body mass index. A mildly raised GGT in someone who drinks four nights a week is a common and informative finding, and it usually improves with a change.
